Plastic Surgery Patient Resources
We welcome the opportunity to meet with you or to provide any additional information you may need. Our normal practice hours are Monday through Friday from 8:30am to 5:00pm.
We can be reached after business hours in an emergency. A plastic surgeon is always available for you after hours for critical questions or emergencies. Simply call the office phone number and your call will be forwarded to the Doctor’s Exchange. They will immediately put you in touch with the doctor.
For your convenience, you may complete the following forms prior to your appointment. Please fill out all 3 forms. The skin care evaluation form is to be filled out, in addition to the other forms, only if you are seeing the aesthetician. When you are finished, please print them out and bring them with you on the day of your appointment.
